    4. Declaration Letter: Your current employer must provide this letter on their letterhead and signed by the CEO. The letter should confirm your employment, their contributions to UIF on your behalf, your employment start date, current maternity leave dates, previous maternity dates, and your expected return-to-work date.

    • UI2.8
    • UI4
    • 1 x payslip
    • Certified copy of your ID
    • Certified copy of the baby’s birth certificate
